A. Introduction

Diabetes is the one of metabolic Disease that related with Insulin deficiency, increase

 

of blood glucose and carbohydrate, protein and fat metabolism disorders. WHO estimate the prevalence of diabetes about 6.8 % in 2025 in Iran. Diabetes is the major reason of Renal disorders, blindness and non- Thrombus amputation.

Persons at risk needs to multiple interventions include: change lifestyle, dietary patterns and drug therapy. Dietary pattern have a major role in prevention and treatment of type 2 diabetes, beside it use of chemical and herbal drugs is usual. 

Many evidence shows that herbal drugs are a wide store of medicine and various structure in their active elements introduce them as a good source of new drugs components. One of the herbal drugs that use in Chinese and Korean traditional medicine is ginseng. Ginseng is the root of a herb that use widely in Far orient for revival of physical, mental and sexual activity and relaxation. Ginseng has antioxidant compounds that can be effective in prevention of inflammatory damage in type 2 diabetes. Limit studies found positive effect of ginseng on HbA1c, because of genetic effect in this disease and no similar studies in Iran we designed this study about the effect of ginseng capsule on glucose and Inflammatory factors in type 2 diabetes patients.

B. Material and methods

55 diabetes subject(20 men, 35 women) by mean age of 50.2±6.2 years were selected, the study was a double-blind randomized clinical trial. Use of usual diabetes drugs like Metformin and Glybanglamid , not to use insulin, age 30-50 years and diabetes diagnosis in 6 month ago at least was include criteria’s of the study and exclude criteria’s were 1.major disease background(Cardiovascular, hepatic, thyroid,   renal and GI   disorders, infections, allergies and chest angina). 2.Regular use of NSAIDs, Warrfarin and diuretics. 3. Smoking and use of alcohol, use of herbal drugs vitamin and supplements, Insulin, hypertension and hyper lipidemia drugs. 4. Pregnant and lactation women. 5. Triglyceride upper than 400 ml/dl   6. Special diet .

Subjects read, understand and sign an agreement about the study and it goals, then inform them about foods and how to use extract and want them to do not change their diet and physical activity along the study. Suitable diet with a distinct Calorie per kg body weight and physical activity designed, clinical and Anthropometrical assessment, dietary intake (food frequency and 24hr recall)and biochemical parameters include: Fasting blood sugar, HbA1c, Cratinin, GFR, Interlukine-6, and hsCRP-α after 12-14 hr fasting and another time after 60 days were collected.

Intervention group receive 4 capsules (each capsule 500 mg) ginseng per day and control group receive similar amount placebo (flour and vanilla) in 2 month. Three 24 hr recall in first and end of 2 month intervention were collected from subjects.

C. Statistical analyses

Scatter of all variants(normal, unmoral)  according to histogram and by means of colograph-esmirnef test were assessed, for compare median insidegroups use repeated mean ANOVA test, by modulating age, BMI, physical activity and weight loss. Use T-test for compare medians between 2 groups in 2 segment, use Mann-whiting test about unmoral distributed variants. Pierson or Spireman correlation was used if needed. The P value less than 0.05 was significant, and SPSS 15 used for analyzed datas.

D. Results

  FBS analytically didn’t change along the study, nonetheless serum density fasting glucose decrease significantly in intervention group compare with control (P<0.001). FBS in control group didn’t  have significantly change along the study.HbA1c % was decrease significantly along the study (P<0.0008)and decrease signtificantly (P<0.0001) in intervention group compare with controls and don’t change in the control group along the study.Cratinine and GFR don’t change in intervention and control group and have no significant decrease along the study and between groups. Inflammatory factors (IL-6, hsCRPα) decrease significantly in intervention group compare to control at the end of study compare to base amount (P<0.02).

E. Discussion

Ginseng Capsule can have a reductive effect on blood sugar and inflammatory factors in type 2 diabetes, hence after more studies with larger sampling and longer periods in Iran , if results were similar can replace or use as a auxiliary drug in type 2 diabetes.
